Other Documentation
For youth needing therapeutic foster care or therapeutic group home services, families and/or guardians may need to provide a variety of documents. Our staff will be happy to explain exactly what is needed to get started! Some of the items you may need include:
- Social Security Card
- Birth Certificate
If it applies to the youth, you may also need to provide:
- Adoption papers
- Proof of Guardianship
- Tribal Enrollment
- Parole and Probation Information
- Court Information — Custody/ Divorce/ Adoption Record/ CFS
