The Area Coordinator
can discuss services and take your referral by telephone.
They will complete an Intake Packet for you that can be faxed,
mailed, or delivered for your or the legal guardian’s
signature. The Area Coordinator completes all requests and
referrals for YDI services needed from any YDI service area.
Our policy is to have a youth in the requested services within
7-10 days of your initial referral date to the Area Coordinator,
or upon discharge date from previous levels of care if the
child is being stepped down. (Note: An unsigned or late returned
Intake Packet will delay services.) |

For STAR Youth Hostel – Intensive Level Group
Home services:
Contact Mikel Wolf, Program Manager; or a STAR Counselor
at (406) 259-2132. These individuals can discuss STAR services,
placements currently available, placements becoming available,
and answer questions. If you request STAR services, a completed
Intake Packet ready for signature and a request for the listed
documents as outlined in items “A” and “B”
above will be faxed to you. Information in “B”
above, or information regarding behaviors and diagnosis, is
need for Peggy Murch in order to complete the prior authorization
paper work. The signed intake packet is needed prior to submission
of Prior Authorization to First Health. Documents in “A”
above are needed prior to placement, unless unable to easily
obtain them. (Note: A Certificate of Need will be required
to submit the Prior Authorization to First Health. Peggy Murch
will complete the Certificate of Need if there is not a Youth
Case Manager. She will need assistance obtaining signatures.
Also, a referral and application will have to be made for
YCM, and this can be documented on the Certificate of Need).
Based on openings, we have accommodated emergency placements.
However, placement of a youth should take no longer than 7-10
days from receipt of the completed Intake Packet and requested
documentation available. |

For Big Sky

be able to assist you. You will need to complete an Intake Packet

* Services can be initiated
with a diagnosis that already exists in your files. However,
due to YDI being a Licensed Mental Health Center, an Initial
Clinical Assessment will be completed within 14 days of the
service start date.
** If the youth currently has a Youth Case Manager, you as
the guardian have the right to choose a child’s service
providers. You can exercise your right to choose by calling
YDI directly or by requesting your client’s Youth Case
Manager to make the referrals and coordinate services. |
